All suite prices are quoted with one bedroom.

Room rates are quoted exclusive of applicable state and local taxes, (currently 9.75%) or applicable service, or hotel specific fees in effect at the Hotel at the time of the meeting.

Hotel Deadline:
To receive the Summit Conference group rates, participants must make their reservations by October 13, 2003. Reservation requests received after the cut-off date will be based on availability at the Hotel's prevailing rates.

Reservation Method:
All reservations must be made individually through the Hotel's Reservation Department by calling 703-418-1234, 800-233-1234 or on Hyatt.com. Please inform them that you will be attending the 2nd National Summit on Patient Safety Research to receive the conference group rate.

Reservation Guarantee and Cancellation:
All reservations must be guaranteed at time of booking with either a first night's deposit or a major credit card. The Hotel's check-in time is 3:00pm. The Hotel's checkout time is 12:00 NOON. In order to cancel a reservation without penalty, it needs to be canceled 24 hours prior to the date of arrival by 4:00pm. Penalty is one night's room and tax.

Taxi and metro instructions:
From Washington National Airport OR the Crystal City Metro stop (blue & yellow lines): either take a taxi (8 blocks, approximately 5 minute taxi ride or 15 minute walk from either point) or call a Hyatt Shuttle (1-888-492-8812) which operates between 6:30am -12:00 midnight and runs every 30 minutes approximately on the half hour to and from the Crystal City Metro and Washington National Airport.
