Request for Open Testimony:

An open invitation is being extended for anyone who wishes to submit written testimony on issues of importance to you. All submitted testimony will be considered in the preparation of the panel session for each of the topic areas. Please adhere to the following guidelines in preparing and submitting your testimony:
  • Address only one issue in any given testimony. If you wish to comment on more than one issue, you may submit a separate testimony for each one.
  • The testimony should be no more than eight (8) double-space pages in length.
  • At the top of the written testimony, please identify the specific issue you are addressing and specify which of the five topic areas of the conference to which it is relevant.
  • We request that you provide your name, affiliation, and contact information so that we may follow up with any questions. If you prefer to remain anonymous, we will accept testimony without this information.

QUESTIONS AND ANSWERS ON DISCUSSION PAPERS AND TESTIMONY (PDF)

Submit the testimony by sending an e-mail to PatientSafetySummit@rand.org with your testimony as an attachment. In the subject line of your e-mail, please indicate "Testimony". Although we prefer an electronic version of your testimony, if necessary you may fax it to the attention of Michelle McMullen-Rider at 412-683-2800. All submissions must be typed.
