Patient Chart Audit Tools with Guidelines

The following documents are sample patient chart audit tools, which can be used to abstract information from the charts of patients with the specified disease. The tools are in Access 97 and Access 2000 as indicated. Guidelines for using the tools are also provided, as PDF files. To view the PDF files, you must have Adobe Acrobat Reader 4.0 or above installed on your computer.

Please note that there are a variety of field types found in the files. These include drop-down menus, input text boxes, tabs, and buttons. In some cases, selecting a tab or button will lead to additional fields or open an additional screen, which may have to be closed to return to the first screen. Also, some fields are mandatory; if they're not filled in, you cannot move forward within the file. Please refer to the documentation for further information about what is contained within the files.
