The following is a list of data collection instruments used in the initial evaluation. The documents are all free, downloadable PDF files. To view them, you must have Adobe Acrobat Reader 4.0 or above installed on your computer.
Asthma Surveys:
All of the surveys from RAND Health are public documents, available without charge (for non-commercial purposes).
Please provide an appropriate citation when using these products. In some cases, the materials themselves include specific instructions for citation. Some materials listed are not available from RAND Health. Those links will take you to other websites, where you will find instructions for use. No further permissions are necessary.
If this is a commercial purpose please send an email with a brief description regarding the use of the survey. We review commercial requests on a case by case basis.
