Nursing Homes Outcomes Initiative

Providers of long-term care use outcomes information for quality improvement, to obtain accreditation, and to provide evidence that they are providing effective care. This document provides a template that nursing home providers could use in developing an outcomes report. In the document, we define outcomes, discuss how they are currently being used in long-term care, and describe the outcomes measures of interest in the long-term care setting. We then provide sample tables and figures that show how to compare outcomes for a specific provider (in this example, the fictitious facility Nursing Center Inc.) to other facilities in the state or the country.
