RAND has undertaken the first systematic evaluation of the use of chiropractic services in the United States. This study has collected a large amount of data about chiropractors, their patients, and chiropractic care, from San Diego, CA; Portland, OR; Vancouver, WA; Minneapolis, MN; Miami, FL; and Toronto, Canada. At each participating chiropractor's office, the following data were collected: (1) a chiropractor questionnaire, which collected information about the practitioner's practice style and education; (2) a patient survey, which collected information about patient's current health status and attitudes; (3) information about number of patients seen in the past week; (4) process-of-care information from randomly selected office records. This report contains the data collection instruments, and a manual for their use.
