Part of a series describing a sample survey and subsequent research on how economic and institutional factors influence birthspacing, family size, and breastfeeding in Malaysia, this report contains the survey questionnaires, recording forms, and interviewer manual used in collecting project data. The longitudinal survey design consisted of three rounds, four months apart. The purpose of each of the eleven questionnaires is discussed.
